The following forms must be completed and submitted by prospective new employees at least 5 days before proposed start date. 

Start dates are usually on the 7th or 22nd of the month. 
 
 
 
 
 
CANDIDATES FOR EMPLOYMENT WITH COLLEGE DEGREES EARNED OUTSIDE THE U.S.:
If you are applying for a position at Hopkins House that requires a college degree, and your degree was earned outside the United States, your degree must be evaluated based on U.S. academic standards prior to employment. A credential evaluation provides a U.S. equivalency for your foreign education. Evaluation services are available through third party providers such as World Education Services (WES). There is a fee for this service. The evaluation report:

•  Identifies and describes your credentials;
•  Verifies that your credentials are authentic; and,
•  Includes an evaluation of the authenticity of your documents.
 
 
 
CRIMINAL HISTORY HISTORY BACKGROUND CHECK:
Federal and state law requires all employees to undergo federal and state criminal history background checks PRIOR to employment in childcare centers. You may apply for a background check online. Click here for instructions on obtaining your federal and state background checks: Background.

FORMS REQUIRED OF ALL NEW PROSPECTIVE PRESCHOOL ACADEMY EMPLOYEES:
The following forms must be completed and submitted if you are hired to work in our Preschool Academy or in another capacity working directly with children:
 
 
MANDATORY PRESERVICE TRAINING FOR ALL NEW PROSPECTIVE PRESCHOOL ACADEMY EMPLOYEES:
Virginia childcare regulation requires that newly hired employees working in state licensed preschools must provide documented proof -- prior to first day of employment -- of completion of TWO mandatory training courses: Pre-Service Training (10 credit hours) and Heath & Safety Training (3 credit hours). These mandatory courses are available online for a nominal fee. Certificates are provided at the end of each training. A copy of these certificates must be submitted to the HR Office as proof of course completion. Employees may not start work without documented proof of training completion on file. MANDATORY ORIENTATION FOR NEWLY HIRED PRESCHOOL ACADEMY EDUCATORS:
Virginia childcare regulations require that newly hired classroom educators working in state licensed preschools must receive orientation around key operational and safety procedures by the end of the first day of employment.  Click here and printout form to provide to the academy principal: First Day
