Background Checks
CRIMINAL HISTORY
FEDERAL and
STATE LAW requires all employees to undergo a criminal history background check, including an FBI fingerprint check,
prior to employment in childcare centers, and periodically
during employment.
The steps required to complete these background checks are described below. You are responsible for paying any fees charged for these various background checks.

Step #1
FBI CRIMINAL HISTORY BACKGROUND REPORT
This background history report is required for all individuals seeking to work with children in the Commonwealth of Virginia.
- Apply for this background check online by clicking here: FBI BACKGROUND CHECK;
- Pay the required $18 fee online by credit or debit card; and,
- Print out the receipt emailed to you after you have completed the online form and submitted your payment.
Step #2
FINGERPRINTS
Fingerprinting is required for all individuals seeking to work with children in the United States.
- Pick up a blank fingerprint card from any Hopkins House location during normal business hours;
- Take the blank fingerprint card to your local police station to be fingerprinted. Hours are limited for fingerprinting and there may be a fee, click on links below for information; and,
- Bring the fingerprint card and emailed receipt you printed out earler to a Hopkins House location within 3 days.
Tuesday, Wednesday & Thursday, 10am to 1pm, only. The fee is $10.
Fridays only, 1-3pm. There is no fee.
Step #3
VIRGINIA DEPARTMENT OF SOCIAL SERVICES BACKGROUND CHECK
This background history report is required for all individuals seeking to work with children in the Commonwealth of Virginia.
- This background check requires you to make an appointment on-line and then travel to a state designated vendor to take your fingerprints electronically. After September 1, 2018, there is $57 charge.
- You will need the following information before you go on-line:
- Fingerprint Code: FPVADSSChDayCntrEmp
- Facility ID: 116808
- Return Report to: Hopkins House
- If you have lived outside Virginia at anytime during the past 5 years, you will need the address(es) where you resided.

Step #4
VIRGINIA CENTRAL REGISTRY BACKGROUND CHECK
This background history report is required for all individuals seeking to work with children in the Commonwealth of Virginia.
- Click here to complete the paper form online: VIRGINIA CENTRAL REGISTRY CHECK;
- Print out the form and sign it in the presence of an official Notary Public;
- Get a $10 money order or cashier's check (payable to "Virginia Department of Social Services"); and,
- Mail the notarized form, with your money order or cashier's check, to:

OUT-OF-STATE CRIMINAL HISTORY BACKGROUND CHECK
This report is required for all individuals seeking to work with children in the Commonwealth of Virginia
who have resided in another state during the past 5 years.
- Contact the state of your previous residence in the past 5 years for information about the process, which varies from state to state. (Do this as soon as possible, since it can take considerable time to complete an out-of-state process.)
